Telephone consultation request form
To request a telephone appointment you need to complete this online form. The details will then be emailed to the team who will assess the request.
You must complete all details for us to be able to assess your consultation request. Before approving a telephone consultation we need to check that the request is suitable and that the images are of sufficient quality. Please do not submit requests for mole checks as moles need to be assessed with a dermatoscope magnifier. Other skin lesions can be assessed with high-quality normal pictures although in some cases a face-to-face appointment will be recommended.
The fee for a NEW telephone consultation is £195 and follow up is £125.
Most insurers will cover the fees for telephone appointments.
